CMS
Cegedim Rx is the first pharmacy IT provider to reach the early adopter stage for CMS and is ready to move forward once it is released by NHS Scotland.
CMS, the final module of the new community pharmacy contract for Scotland, is now available for Cegedim Rx customers to order. The chronic medication service will allow patients with long-term conditions to register with the community pharmacy of their choice for the provision of pharmaceutical care as part of a shared care agreement between the patient, Pharmacist and GP.
The main aims of CMS are to introduce a more systematic way of working and to formalise the role of community pharmacists in the management of individual patients with long term conditions in order to assist in improving patients’ understanding of their medicines and optimising the clinical benefits they gain from their therapy.
Pharmaceutical Care Planning
CMS enables pharmacists to take a more active part in the treatment of patients. Initially pharmacists will assess registered patients to identify and prioritise individuals or patient groups with unmet pharmaceutical care needs in order to target patients most in need of their support. Pharmacists will then work with patients to identify and record the patient's pharmaceutical care needs, care issues, any desired outcomes, and actions required to deliver these outcomes. These will be documented in a pharmaceutical care plan which will be reviewed by the pharmacist and patient at agreed intervals.
Shared Care
Once a patient is registered for CMS their GP is notified electronically. The GP can then agree to enter into a shared care arrangement with the pharmacist and patient. This includes the option to generate a serial prescription which can run for 24 or 48 weeks. The pharmacist will supervise the patient over this time period using the patient's pharmaceutical care plan, sharing relevant information with the consenting patient's GP. When the serial prescription has been completed the GP will then review the shared care arrangement and decide whether to update this or regenerate the prescription.
