Cegedim Rx were delighted to present Glasgow City HSCP (Health and Social Care Partnership) the ‘Working in Partnership’ award at this year’s Scottish Pharmacist Awards. The honour was awarded to the team for the excellent partnership they created with the 'Improving the Cancer Journey' team and the 'South Lung' team at the New Victoria Hospital in Glasgow.
Partnerships are an essential part of the healthcare industry. Executed in the right way and with the right partners, they can bring much needed improvements to the way in which healthcare is delivered in the community. As a new partnership, which focused on transforming the delivery of care for those living with lung cancer, Cegedim Rx was honoured to recognise Glasgow City HSCP.
Working collaboratively, they have developed a new referral pathway which enables patients with lung cancer attending the 'South Lung' team, to be referred for a pharmacist medication review within their GP practice. This is a great example of how joining up healthcare is a key way to achieve the best outcomes for patients and ultimately advance care.
The award was collected by Lynn King, Practice Pharmacist at Glasgow City HSCP, who said: “We were surprised and delighted to win this award. It recognises the hard work and dedication of our 'Prescribing Support' team in forming new partnerships and finding new ways to utilise a pharmacist’s professionals skills for improving patient care.”
As the primary leader of this partnership, Lynn added, “This work shows how you can effectively improve care through intra and inter-professional working across not only primary and secondary care, but also the health and social sectors. The willingness and commitment of the 'Prescribing Support' team to become involved in a new clinical area, along with the enthusiasm and dedication of all the partners involved, has been instrumental to the success of the pilot. The challenge now is to consider if this model can be rolled out to include other cancers and to be extended to other areas within NHS Greater Glasgow and Clyde.”
